will be forfitted.
Surgical Fees:
All professional fees for cosmetic surgery are due two weeks prior to surgery. Our surgery scheduling/financial
consultant will discuss payment with you.
Re Op Fee's: If any revision procedures are necessary there may be a surgeon's fee, however, any resulting hospital cost,
supplies, implants and anesthesia would be the patient's financial responsibility. The surgeon's fee quoted herein is
offered for a six-month period only. All other fees are current estimates only.
Separate Fees:
The following fees are separate from the doctor's professional and surgical facility charges:
- Anesthesiologist's Fee. This is payable at the time the surgical and operating room fee for cosmetic surgery.
- Hospitalization fee.
- Prescription medications, outside lab work, x-rays, pathology, and extra post-operative supplies.
